Insurance is not required in order to make an appointment.  If you do not have insurance coverage, the cost of a new patient visit is $125 and is payable at the time of your appointment.

Your first visit to Evans Dermatology Partners involves a few special steps so that we can get to know you. To understand what to expect, please read through this page.

Insurance Information

If you would like us to submit to your insurance company, please bring a copy of your insurance card and a photo ID with you to every appointment.  The information on the front and back of your insurance card is needed in order to accurately bill your insurance company.

Co-Payments and Insurance Deductibles

Co-payments are due at time of treatment.  For your convenience, we accept cash, checks and credit/debit cards.

Many insurance plans require that policyholders pay a deductable and/or co-insurance.  If you owe payment against your deductible or as part of your co-insurance, we will send you a bill after your insurance company informs us of the amount you owe.

Treatment of Minors

Whenever possible, a parent or guardian should accompany minor patients to their appointments.  However, we understand that occasionally patients may be brought to their appointment by another adult, or teenagers may come to an appointment unaccompanied.

When this occurs, we require the following form to be signed by a parent/guardian in order to authorize the provider to see the patient.
